Check In Requirements:

• On the day of your first appointment, please bring:  1) Current Identification (Drivers License or State ID) and 2) Current insurance card(s)
• You will be required to pay any copays and any outstanding balances on the same day of your appointment.
• If you do not have insurance you will be asked to pay the reduced out-of-pocket fee for services on the same day of your appointment.

Understanding Your Benefits:

• Know what your health insurance covers.  We will help you to verify your insurance coverage.  Sometimes the informtion we are told by your insurance is not correct or too vague to know your exact coverage.  Therefore, it is always up to you to confirm your exact coverage, as you are ultimately responsible for your bill.  Simply call the customer service number on the back of your insurance card and and ask then for your coverage.
• We are providers for most insurance companies, but you will still want to verify what your specifc plan covers for the service at our clinic.

Payment Responsibility & Payment Methods:

• Healing Choices will file your insurance claim as a courtesy, but in the event that your insurance does not cover our services you (as the patient) have full responsibility for payment of your bill.
• We offer the flexibility of paying by cash, personal check, and major credit cards.
